Opt-out setting for entirely disabling all automatic refresh

Bungie.net's caching is very disruptive and I can't come up with a way to make auto refresh work better with people's playstyles. So let's just have a top-level opt out of auto refresh that will disable any and all cases where DIM would automatically refresh profile data. With this enabled, only clicking the refresh button (or equivalent) or the initial page load would refresh profile data.

As part of this we should have some indicator in the tooltip that auto refresh is disabled.

absolutely miserable when the auto refresh triggers right before changes to your actual inventory, you have to move stuff around while accounting for ghost items.

in some cases DIM also does something bad where it will take a cache from the API inventory as gospel due to its timestamp and roll back your inventory by a couple minutes and this always happens with a throttle lockout which is just a kick in the balls since the inventory was in a correct state just a second ago.
