Scaffold: fetchWithL402() interface + mock 402 server

Add a minimal Node/TS scaffold:

• export fetchWithL402(url, init, opts)
• implement mock server that returns 402 + invoice metadata then accepts retry with proof header
• tests covering retry path

No real payments; mock invoices.
