Crashes Frequently

Every 10 minutes or so it randomly crashes to desktop. It leaves behind an error log and crash dump every time. It also crashes when I try to exit from within the game.

The error logs are all something like this:

Unity Player [version: Unity 5.4.3p3_ffc313269cf8]

YookaLaylee64.exe caused an Access Violation (0xc0000005)
in module YookaLaylee64.exe at 0033:d1b963b2.

and then a big data dump.

I think this is related to using multiple monitors in "extend" mode. If i change to a single monitor before launching the game I don't get crashes.
