Add intensional filters for ICD-10-CM code system

It would be useful to have a way to specify all codes "under" a partial code in the ICD taxonomy. For example:

codes under S86 = all codes related to injury of muscle, fascia, or tendons of lower leg
code under S86.0 = all codes related to injury of Achilles tendon
