Survey setup: Allowing users to share email addresses of others

After selecting "Yes", this doesn't change anything for the respondent's survey right now. Instead, when "yes" is selected, a question should appear (on last page of respondent survey) that says:

"Is there someone else who you think should take this survey? Enter their email address here."

With help text: "Be sure you have their permission to share the email address".

Field should be open-ended short text, with buttons to add or delete multiple email addresses.
