Issue with showing a multi-select list of check-boxes

Hello!

I'm attempting to render a questionnaire that has a list of non-mutually exclusive choices that I want to show as a single group, where the user can select zero, one, or more values from the set of choices. I'd to display it as a list of values and checkboxes, where each value can be selected and unselected.

Here's an example of what I'm trying to accomplish.

I've attempted several different approaches and below you'll see my attempt to hack it without success. Can someone point me in the right direction of how/if this can be accomplished? Huge thanks in advance!

It looks like you've created three items, when you have just one question with three answer options. Put the three answer options together in one answerOption array, set item "repeats" field to true (multiple answers can be selected), and then you don't need the containing group item.

Since you are using the choice type for your answers, you could also try using the NLM Form Builder to build the Questionnaire. (If you pick type "choice", and request an answer list, and toggle the "Allow repeating question?" to "Yes", then you will get an "Answer list layout" question that lets you pick "checkboxes".)
