After initial camera permission grant, app crashes; works on next launch

• There is an issue after initial granting of camera permission. Likely we need to handle this initial case specifically and e.g., not start streams until permissions are granted.
• consider: expose public api for requestCameraPermission so clients can customize control of permission grant flow
