Features to compose buffer

I have used the compose buffer for some time and I have a couple of features there could be nice to have:

- Allow to launch it in insert mode, so its possible to start writing as soon as the buffer launches.
- Allow to "leave" insert mode when you send the composed buffer to the target.
- Resolve the {placeholders} in the buffer
- Change the title of the compose buffer. It should be per-call
