Ravenous - spawn random food props, pick up to heal/gain buffs
Spawn a trigger on top of the food instead of manually picking them up (scripted buttons dont tend to work very well, could try script_button entity)
White Weapons / Items - Weapon / Items picked up on this level are less effective, e.g. -20% DMG / Healing

Monstrous Hocker variants - Damage briefly decreases survivor DEF(?)
Monstrous Retch variants - Damage briefly decreases survivor move speed
