external account provider: form is not empty when we add a new provider

Once we have set a new ldap external account provider the modal is not empty if we reopen it
Steps to reproduce

• set an external ldap provider
• add a new provider to it
• try to add again another provider
• the form is not empty

Expected behavior

I expect the form is empty and we do not have the previous value

Actual behavior

we can see the previous value and the state disabled when we have saved the form

verif the bug is not more reproducible

• add an external account LDAP provider
• once set add new providers to the first provider -> the UI is reloaded and you can see the the new provider
• press again on the button add new provider -> ther field are empty without the previous values
