Add support for two effects simultaneously / volume column fx

Parsed song data will contain information on volume column effects or "`effect2`", but the player does not do anything with them even though most/all are shorthands for existing effects. 

This also allows notes to have two simultaneous effects, however the current code is built around cells only having either one effect or none.
