Generate multiple versions of the PDF document

Currently, only the tip of master is rendered in PDF. We could generate a PDF for every tagged version. We could use the git tag for the version identifier. We could create subdirectories in the github.io pages.

Clarifying - this means people can only download the questionnaire for 2.0, right?
