Subject: [PATCH] Limit Form Sting edits

Edited Limit Form strings to match other forms and to use the attack's original names for Sonic Rave, Last Arcanum.

Also starting to fix width issues; started small with changing memory card slot names to match Re:CoM cuz apparently, that's what's based on (?)
