Distorted input field for maximum number when creating cash entitlement

Describe the bug

Maximum number input field is not aligned properly

To Reproduce

1.Make sure OpenSPP cash module is installed in your OpenSPP instance.
2. Go to OpenSPP and create a program with cash as entitlement manager
3. Open the created program then go to its configuration tab and click on entitlement manager
4. once the modal for cash entitlement manager ids displayed, click on Add a line.
5. notice in the displayed modal, the maximum number does not align with its input field
6.
Expected behavior
Fix alignment issue for maximum number when clicking add a line for cash entitlement.

Test Steps:

Current behavior:
The maximum number field is now aligned when the create entitlement modal is opened
