Enhancement to Change Request module

Modify the Change Request base module to make it independent of the OpenSPP base module

Business Requirement

• The sl project requires a separate instance for the Fertilizer Distributor Registry. This instance must not have OpenSPP installed.
• The current Change Request base module depends on the OpenSPP base and other modules. When used in the Fertilizer Distributor Registry, OpenSPP will also be installed.
• The Change Request must be modified to be independent of OpenSPP. This is not just for the sl project but the primary goal is to make this module usable in other implementation scenarios and at the same time maintain its compatibility with existing Change Request type modules for SPMIS and Farmer Registry that depends on it.
