Add Issue & Pull Request Templates

To keep all Vault Web repositories consistent, we want to introduce unified Issue Templates and a Pull Request Template across every repo.

Description

• Add .github/ISSUE_TEMPLATE/ with:

  • bug_report.md
  • feature_request.md
  • config.yml (optional)

• Add .github/pull_request_template.md

Goal

Ensure all repositories follow the same structure for reporting issues and submitting pull requests, improving collaboration and maintainability.
