Enforce unique identifiers for Object Types, Attributes, and Instances

A unique identifier would be like ObjectTypeCode, AttributeCode, and InstanceCode. I dropped them from an earlier design because of increasing difficulties to the user to create them in the Excel Template but they will be needed to maintain unique entries as required by HydraPlatform
