Scaler Configuration Updates

Need to change to enable the standard set of channels.
As a result, found that the enable PVs were not being generated correctly.
Also need to remove the Continuous Scan PV items ... it's not needed and gets in the way.

This should be done in #94 already.

The changes I made there are:
-- removed the Continuous Scan PV
-- For channel Id < 10, add a "0" when generating the PV name

Regarding the standard set of channels, do you want to have all 32 channels configurable? If yes, it is a simple changes in AMDSCentralserverSGMScaler.cpp.
