Plugins being added to courses when trying to remove them from the tool

What happened?

I've created a new course, only including a text component and tried to preview, the course gets stuck loading with console errors about missing config for plugins not included in the course. When I export the course there's a number of plugins that I've not added to my course but also are not on the tool at all.

If I import one of the plugins that's missing from the tool (but appearing in my course), and export my course again, the plugin is no longer included in the export. However if I then remove the plugin from the tool through the plugin management page and export my course, the plugin that's just been removed is included in the export.

Steps to reproduce

1. Create a new course using just a text component
2. Export the course, if no plugins have been removed from the tool only the text component should appear
3. Remove a plugin through plugin management (doesn't matter which one)
4. Export the same course and now the plugin that's just been removed from the tool will appear in the export
5. Impot the plugin you've removed back onto the tool
6. Export the same course again and the plugin you removed/added will now not appear in the export

I've been unable to replicate this one locally - have tried adding/removing plugins from a course + tested whether plugins from other courses are erroneously added - both uneventful.

This could be something that's come about due to a botched upgrade process (or a problem with the upgrade process), or there may be a bug with how we maintain the list of enabled plugins per course. Will leave this one open for now so we can do a bit more testing.
