Support redirect for posts that have been moved

Similarly as in the docs site, if a page is moved (e.g. date or title changed), old references are not valid anymore. It'd be nice to add support for HTTP redirect for such cases.

Page A ---> Renamed to ---> Page B

Requests to Page B: OK
Requests to Page A: HTTP redirect to Page B
