Show graph badge on loops that are part of a workflow

Summary

When a loop is part of a workflow graph, show a visual indicator (badge) in the loop list so users know it's being orchestrated.

Acceptance Criteria

• Loops with graphInstanceId show a [G] or ◆ badge in the loop list
• Badge is colored to match graph status (running=cyan, completed=teal)
• Hovering or selecting shows graph name in detail pane
