[BUG] IDE connection inconsistency: /ide shows connected but /doctor missing IDE status and intermittent warnings #16394

What's Wrong?

Running Claude Code from VSCode integrated terminal shows inconsistent IDE connection state:

  • /ide shows VSCode as connected
  • /doctor output has no IDE section at all
  • Intermittent "IDE not connected" warnings appear at startup and randomly

What Should Happen?

  • /doctor should show IDE connection status
  • No "IDE not connected" warnings when /ide confirms connection

Steps to Reproduce

  1. Open VSCode with Claude Code extension enabled
  2. Open integrated terminal (Cmd+`)
  3. Run claude
  4. Run /doctor - note no IDE section
  5. Run /ide - shows VSCode connected
  6. Observe intermittent "IDE not connected" warnings
