Add confirmation output when skills are extracted

Problem

When Claudeception extracts a skill (especially auto-extractions), there's no indication to the user that:

1. A skill was created
2. Where it was saved
3. What triggered the extraction

This makes it hard to know when the system is actually working.

Proposed Solution

Add a mandatory "Step 6: Confirm Extraction" to the extraction process that outputs a confirmation block:
