Instruction following mega-issue in Claude Code / Cursor

Have noticed a handful of issues w/ instruction-following off of the base CLAUDE.md / main-kurt.mdc cursor rule file:

1. Failing to identify when the user is attempting to add a new format template
2. Failing to update the plan.md with progress within a project
3. Making a single attempt at source gathering (via external research or internal content list) and moving on if nothing found (rather than fanning out to other query variants).
4. Source gathering output is not clearly defined, so the agent freelances (creates random /project/research/ files, rather than writing to plan.md or a project/sources.md with citations.
5. Agent is confused about separation of document metadata (in db) + actual source docs in filesystem. Need to explicitly require search / list / get via kurt rather than grep.
