Create a GitHub issue template for the request of a new extension.

We need to make sure that:

• no duplicate request exist
• we ask the name of the extension
• the URL of the project
• the URL of the website
• a short description of the extension
• the link to the main LICENSE
• a checkbox that the license complies with the list of allowed licenses
