Bug/Feature: Continue Agent mode should not stop on MCP tool failure

Validations

• I believe this is a way to improve. I'll try to join the Continue Discord for questions
• I'm not able to find an open issue that requests the same enhancement

Problem

If you have an MCP installed the Agent completely stops on any tool failure

Including

• Miss-configured JSON from the Agent Tool call
• MCP Server returning an MCP error

Solution

In both cases Agent mode should continue iterating on the tool. In many cases the error response has enough context for the LLM to fix the MCP tool call error.

@uinstinct this would be a good one to work on next week. I would +1 the feedback from @etherandrius , if it's an error we should continue, which is currently how our built in tools behave I believe. So I think this is an MCP issue.
