Missing templates for 2fa/authenticate #146

Hi!
It appears that once I login to an account with 2FA enabled, there is not a template for entering the 2FA codes,
Tried with version 1.5.1 then upgraded to 1.7.1 for same result
Let me know if you need anymore information!
