Not able to implement custom header with page number in PDF

I am trying to create a code for Printable PDF format with multiple pages and repeated header with page number, I checked code and index.js other option, but either I missing something or there is any way to implement this with plugin option but I am not getting it, anyone can please suggest the solution.
