AutoDND not turning off on app exit

With the new update, AutoDND does not stall at startup anymore (and does not need to be reconfigured when the phone is restarted #13) (thank you), but it doesn't turn off DND with the app anymore.
This is true for both simply going to another screen(home/other app), or dismissing the app from recents screen.

I have tried restarting my phone as well, issue persisted.

Some stuff to confirm first:

• Are you using AutoDND with Shizuku?
• If the answer of previous question is yes, run the following command (at least) 2 times (one when using your target app, and another when you're at launcher/desktop) using adb shell or whatever terminal with root access:
  dumpsys activity activities | grep mFocusedApp | sed 's/.*\\ u.\\ //;s/\\/.*//' | grep -E -v 'null$'

Actually, the main problem is accessibility sometimes crash because too many apps need accessibility permission.
using usage acces permission more stable compare to accessibility permission. But cons, need notification to work properly. reference
