Issue in Product review form

Captcha not showing in product review form and when i click on submit button its showing below notice

Notice: Undefined index: recaptcha_challenge_field in app/code/community/Fontis/Recaptcha/controllers/ReviewController.php on line 36

and also captcha not showing in review page i am using ultimo theme do i need to replace template and layout files to my theme
