Optional phone number

Description:
Right now the only way people are able to contact others are by email (we grab this email from their accounts when they log in).
We can have another step within the create modal that asks for what type of contact setting they would like.
They can pick between:

• Email
• Phone
  This chosen form of contact should replace the contact info when a user clicks on "View Contact", it currently only shows email.

Goals:

• Update the create modal form that we currently have to allow users to select form of contact
