Not fully compatible with Zabbix 3.0.X

When used with Zabbix 3.0.X (was released in February), hosts are shown as "undefined"

I can confirm this. Zabbix 3.0.4 here, hosts shown as "undefined"

Otherwise great and very useful plugin! Would be nice to have that fixed.
