The Preface remarks that the text is “open access” and “open source,” but there is no license that undercuts copyright. Therefore, I believe that — at least in the USA, and probably in some other jurisdictions — it is by default in copyright, and, in particular, modified versions may not be distributed. That it's open access and that we can see your source code don't change that.

Would you consider making permission to copy and distribute explicit?

I agree. The lack of an open license will hinder adoption.
