Can I get a short copyright notice?

I borrowed and heavily reworked this widget into Volt templates and a commonjs module.  I'd like to be able to put the copyright notice into the files.  Can I get a short about 3 line attribution comment?

I added a config dump panel, ctrl-shift-d toggle and in local storage it remembers which panel was open or if no panel was open.
