Cannot toggle live preview mode + many icons appearing on the tab title bar

Hi i have a couple of problems

• cannot toggle the live preview mode

• there is a strange thing that happens, is there will be icons on the tab title that keeps adding when the plugin is enabled and if i open new tabs. the button says "Action", and will show a search window and sometimes it just will not do anything.
  this is definitely from your plugin because when I untoggle it, those will disappear. is this a known issue?
