Config options not available in UI #5

Including the moderator's name in the email notifications should be optional (configurable) for privacy reasons
This is now configurable.
I see the include_moderator_in_emails config in the code, but it's not visible on Moderate/config page.
Users with access level lower than the moderate_bypass_threshold will never be able to add attachment to other people issues (see thoughts on trusted users below)
They will be able to attach once they qualify for auto-approval - e.g. when adding notes to their own issue or when adding entry N+1 where N is the threshold.
I see auto_approve_threshold in code, but it does not seem to be configurable.
