Investigate track end/start off by one issue #31

More of a note to self to confirm this is an issue, and investigate fixing it. As one example, see the end of track 1/start of track 2 for http://redump.org/disc/44778/
This also might be a load-bearing issue for the P channel data like it was for the index, but still need to investigate that too.
