De-duplicate "intermediates to be revoked"

https://bugzilla.mozilla.org/attachment.cgi?id=9005280 from Bug 1487485 shows several duplicated lines for intermediates that have the same serial number and issuer. There aren't duplicates in Kinto, so it's probably an output duplication that we should resolve.
