[Patch] Add MIDI CC Output #6

Hello.
I need to make sidechain control. I try to send measured (then scaled/shifted/limited) values as MIDI CC packets. I prepare patch for it. Hope it will be useful.
Example of use of patched version with Carla: Tested with Dry/Wet knob control to make Reverb or Echo plugins sidechained (modulated to reduce the volume of the effect whenever the vocal is present): "Add JACK application" -> jack_meter -w 0 -f 25 -m 0, Audio inputs 1, Midi outputs 1. Then connect it all, as usual.
