feature request: Make git-gutter+-refresh an interactive command

When I commit changes outside of git-gutter+, it doesn't know about them. I could make a change and save the buffer to get a refresh, but it would be nicer to not have to. For now: `M-x eval-expression (git-gutter+-refresh)`.
