Entity extraction produces hallucinated results when distilling a thread

To Reproduce
Distill the included thread:

JWT algorithm mismatch in Spring Security_2026-04-15T10-45-23.json

The extracted entities listed - "AI, Autopilot, CUDA, Dr. Anna Petrova, GPU, HPC, NVIDIA, Python, TensorFlow, Tesla, University of Alaska, battery technology, electric vehicles, satellite data, self-driving cars, and statistical models" do not appear anywhere in the thread.

Thanks for reporting this. I’m doing a full issue sweep today so recent bugs do not sit without a maintainer response.

I’m keeping this open for now rather than guessing. It stays in the active triage set until we either confirm it is already fixed in recent work or turn it into a concrete implementation task.

I checked the current thread-distillation path directly rather than assuming it was the same as the older memory-quality bug family.

My current maintainer read is: this issue should stay open. I do not see enough evidence yet to call it already fixed, and I do not want to close it away with a guess. The remaining work here is not just parser cleanup — it is making sure the entity-extraction path has the right evidence and validation discipline before wrong entities are written.

If you have a concrete current example where the extracted entities are clearly wrong, the best follow-up would be:

• current app version
• a short description of the source thread topic
• the obviously wrong entities that were produced
• whether the problem happened in preview, final extraction, or both
