Standardize phone number format

Some phone numbers are entered as a string of ten numbers (e.g., 7037125083), rather than breaking it up with hyphens (e.g., 703-712-5083). This is resulting in a failure to detect duplicates, since the sc_id is an MD5 of the lobbyist's name and phone number. Enforce a hyphen-ful format.
