Non oper flood protection

I'm looking for a module which allows to punish a non-oper user trying too much access to OperServ.
The idea is that if an user get 3 (settable in conf) denied access to operserv in a reasonnable delay (1 minute?), an action is performed (ignore, kill, shun, ...) with a warn message (or reason if killed/glined)

I too would like to see something like this, but i would also include nickserv (failed identifys), as well as attempted use of global and/or other services/commands that require being an irc operator without proper authentication first
