Add optional marker to :instructions to specify position of generated commands

#14 introduced support for arbitrary :instructions. It would make sense to extend this to allow a marker in the sequence passed to :instructions, maybe a keyword like :content, that could optionally be used to specify where the generated instructions should be inserted.
