Case indent: use cinoptions : and = values

:N    Place case labels N characters from the indent of the switch().
            (default 'shiftwidth').

=N    Place statements occurring after a case label N characters from
            the indent of the label.  (default 'shiftwidth').

(Same as default cindent)
