Enhance key mapping configuration

• Enable CTRL modifier in key mapping.
• Enable key mapping in dialog.

I'd like to use same key mapping (e.g. CTRL-j/CTRL-k) to scroll one line and to select item in each dialog.

Support for CTRL modifiers is a good idea. It will enable us to implement extra actions that might be useful.

For example, in issue #65 @seefood complained about DOWN being interpreted as "go to the next link or scroll down one line". He said it feels unnatural, because sometimes you press the down arrow and you end up going to the next link which is to the right.

So, it would be good to have more actions such as ABS_DOWN/ABS_UP (scroll down/up one line, ignoring links), or HALF_PGDN/HALF_PGUP (scroll down/up half a page) as you requested in issue #69. But there are only so many keys on the keyboard, so the CTRL modifier must be enabled if we want such actions.

Proper actions for dialogs are also a good idea. For the time being, dialogs reuse the standard actions: UP, DOWN, OPEN, etc. There's also an undocumented QUIT action for closing the dialog, mapped to ESC and CTRL-C by default. This is neither complete nor orthogonal.

BTW, input prompts don't have actions, they just interpret ENTER as "confirm" and ESC or CTRL-C as "abort". But this is OK, in my opinion.
