Allow Authentication for External Terminology Services

Blaze can already use an external terminology service by setting the env variable EXTERN_TERMINOLOGY_SERVICE_URL to a FHIR base URL of a terminology service. However an authentication isn't possible.

We need:

• Basic Auth
• OpenID Connect Auth with Client Credentials Flow
