@katsanevakis some questions:

• In the "Center" field, we should return a list of all the Hospitals?
• The field "Patient Hospital ID" is a patient id specific to this hospital?Does it have any logic/validation?
• In the Personal Details, there is a "Patient ID:2". We think this should be removed since the patient has not yet been created.
• We need to make which fields are mandatory clear.
• Do we need any validation on the phone fields or anywhere else?

• Yes, this is a dropdown similar to the one currently displayed
• It is a unique number for the specific patient and hospital combination. It will be manually submitted by the Doctors

@katsanevakis there are 3 Name fields. Are all of them mandatory?

@dkoutsonikolis There were 2 options for the Name field Handling

1. Single Name field, just like the current build

2. Separate First Name, Middle Name, Last Name fields. If we go with this approach, then only one of First Name or Last Name should be compulsory.

Option 2 is preferred by the Stakeholders but if it requires additional effort option 1 is acceptable as well.
Please advise on the complexity of these options, so that we can update the designs if needed.
