PDF UA-1 compatible document doesn't expose headings correctly?

It got various headings at level 1, 2 and 3, but none of them are currently accessible with h / shift + h in Paperback. Dunno why, but since UA-1 is the official accessibility extension of PDF, I thought it might be important to have proper support for that one in Paperback as well.

I just noticed that lists / list items don't get exposed properly as well, so that might be worth checking out as well.

@Timtam @trypsynth I can also duplicate this. It's worth noting that Bookworm can't detect headings for this PDF file either.

PDFs don't currently support headings in Paperback at all. I think pdfium does support this though, and I want to do it.

@trypsynth That would make PDF files much more accessible. And speaking of PDF access, could you please do something for #101 for broken paragraphs? IMO this is the most serious issue currently affecting PDF files in Paperback, something which prevents us from navigating them by paragraph.
