[BUG] Circuit Python Serial Monitor disappearing

Sometimes with the Circuit Python Serial Monitor terminal panel is closed / deleted, it will not reopen. Clicking the "Open Serial Monitor" button or using the "Circuit Python : Open Serial Monitor" doesn't cause an obvious errors, but it does not open a new terminal window. Once it gets in this state, the only way I've been able to access the Circuit Python Serial Monitor terminal window is to restart VSCode

I'll try to better identify the conditions when this occurs again.
